A search result described a Health Plan Advisory 26-06 changing ABA service definitions effective July 1, 2026. It could not be fetched. Treat the service definitions and provider qualifications below as the 2015 and 2016 plan text, and confirm against the current plan and advisory.

Is ABA covered by Nebraska Medicaid
Yes. Nebraska added Behavior Modification Services under EPSDT, approved "as of March 29, 2016, with an effective date of October 1, 2015." The plan defines ABA as "the process of systematically applying interventions based upon the principles of learning theory to improve socially significant behaviors" and states that "all three-treatment models may be incorporated into the behavior modification services" (CMS, SPA NE 15-0013).
Who is eligible
Services are "available to all Medicaid EPSDT eligible clients with significant functional impairments resulting from an identified ASD and/or a DD diagnosis as defined by Nebraska Revised State Statute 83-1205." Medical necessity "shall be determined by a licensed physician, licensed psychologist or, a licensed independent mental health practitioner (LIMHP)." An Initial Diagnostic Interview is "completed prior to service provision," and may be performed by physicians, licensed psychologists, provisionally licensed psychologists, and LIMHPs (CMS, SPA NE 15-0013). SPA 16-0007, effective July 1, 2016, also allows "Board Certified Behavioral Analysts, Licensed Independent Mental Health Practitioners, Licensed Psychologists, and Provisionally Licensed Psychologists to provide the Functional Behavior Assessment" (CMS, SPA NE 16-0007).
Rates
"Effective August 1, 2025, Nebraska Medicaid's applied behavioral analysis (ABA) rates were adjusted in alignment with ABA rates from Medicaid agencies located in states with geographic proximity to Nebraska. Nebraska Medicaid's adjusted rates reflect a blended average of these nearby states' Medicaid rates for ABA services." The rates "will be the same for governmental and private providers" (CMS, SPA NE 25-0020). Per-code rates for 97151 to 97158 were not retrieved, because the Department's fee schedule page was unreachable.
Place of service and telehealth
Community Treatment Aide services are "supportive interventions provided primarily in the client's natural environment," which is "primarily the client's home but may also include a foster home, school, worksite or other appropriate community locations." Day treatment is "a community based, coordinated set of individualized treatment services." Behavior modification services may be provided "through telehealth technologies, excluding services requiring 'hands on' professional care" (CMS, SPA NE 15-0013).
Prior authorization
The prior authorization process, the role of the Heritage Health managed care plans, and any hour limits were not verified.
Provider qualifications
A BCBA "may strictly provide outpatient assessment and treatment as part of ABA services only," "shall have a master's degree in behavior analysis and be board certified by the Behavior Analyst Certification Board," and, when strictly providing ABA, does "not require supervision." A BCaBA needs a bachelor's degree in psychology, social work, child development, or a related field, the equivalent of one year of full-time work experience or graduate studies in direct child, adolescent, ASD, or DD services, and must meet BACB certification qualifications. An RBT must meet BACB certification qualifications with an educational and experience background set out in the plan. "The RBT and the BCaBA must be supervised by a BCBA" (CMS, SPA NE 15-0013). The plan text predates Nebraska's behavior analyst licensure, and whether licensure is a Medicaid enrollment condition was not verified. See the Nebraska Licensing page.
The commercial mandate
The commercial autism mandate text, age cap, and limits were not verified, because the Legislature's statute page was unreachable.
Practices with in-house diagnostics versus ABA-only practices
Under the 2015 plan text, the Initial Diagnostic Interview and the medical necessity determination come from physicians, psychologists, or LIMHPs, while a BCBA may perform the functional behavior assessment after SPA 16-0007. A practice with an in-house psychologist or physician can keep the interview and the necessity determination inside the practice. An ABA-only practice relies on an outside professional for both.
